Sources of Strength (SOURCES) Elementary Interest Survey

The KY Division of Behavioral Health is conducting an assessment of elementary schools that are interested in learning more about the new Sources of Strength (SOURCES) activity-based program for elementary schools, grades 3-6.  The elementary model continues the developer's commitment to move even further upstream to increase health and wellness, and decrease negative downstream outcomes to an even younger age group.  Like the middle and high school model, the elementary model is designed to empower individuals and communities full of connection, belonging and resilience.

This 10-question survey is designed to measure the level of interest in the elementary model.  It is not a commitment to implement the model at your school, and it is not a commitment from DBH for funding to implement the model.  Information gathered here will be used to determine the amount of interest from KY elementary schools so that DBH can provide additional information to interested school staff.

* 8. The SOURCES Elementary Program is available for grades 3, and 5, and for 6th grade if it is included in your elementary school.  Activity-based curriculum is grade specific. Schools decide which grades (one or more) in which they will implement the curriculum. Making no commitment, please provide the number of CLASSES PER GRADE that exist at your school.

The first training of Coaches is planned for Jan 26-27, 2021, with a second training scheduled for late summer or early fall 2021.  Coaches are the people who train classroom implementers at a school.  Once coaches are trained, they can train for implementation to begin at a time of the school administration's choosing. The model is flexible in terms of implementation of activities and sessions over the course of the school year.  This program is about building skills over time. If multiple grades are implemented, the effort continues as students move to the next grade.
