Through a Screen Darkly: Psychoanalytic Reflections During the Pandemic 1st Edition, Kindle Edition

5.0 5.0 out of 5 stars 7 ratings

Winner of the 2022 Gradiva® Award for Best Book – Historic Moment for Reflection!

This book offers real-time, intimate reflections on Dr. Friedberg’s patients as they struggle with COVID-19 and its disruptive, dispiriting fallout.

Through a Screen Darkly identifies the psychological distress caused by the pandemic, examining how the particular elements of COVID-19 – its ability to be spread by those who seem not to have it, its intractability, the long-term uncertainty that it engenders – leave even relatively stable people shaken and unsure of the future. The book examines how, amidst radical uncertainty and the prospect of massive social change, such people learn to become resilient. The main theme of the book is that, of necessity, we learn to adapt. Though we still can only see "darkly," we can call on the resources that we have, as well as those we can reasonably acquire, so as to retain a sense of our dignity and purpose. Through a Screen Darkly examines what is possible now as the pandemic runs its course. It makes no predictions of how all this will ultimately play out, but offers a time capsule of how people have coped with a disease that landed suddenly and that we still do not fully understand.

Offering a series of intense encounters with worried, traumatized people, this book will be invaluable to in-training and practicing psychiatrists, as it points to the several possible directions for our national, psychological recovery from the pandemic.

Review

I have wholehearted admiration for this extraordinary volume by noted psychiatrist and psychoanalyst, Ahron Friedberg. It should be of great interest to psychiatric patients and psychiatrists alike. It could also be subtitled "love in the time of the COVID pandemic." The pandemic crisis transformed the close face-to-face therapeutic situation into a remote screen-to-screen encounter. As the pandemic is both danger and opportunity, the dark screen, paradoxically, shines a bright light on both therapeutic participants. Enlightened by his own struggles with the pandemic, Friedberg compellingly renders the existential dramas and unique narratives of his patients and teaches them to cope with the crisis, to maintain hope and resilience, to cultivate new contacts and connections, to discover new meanings in their lives and loves.

Henry Zvi Lothane MD, Clinical Professor of Psychiatry at Icahn School of Medicine at Mount Sinai.

Through a Screen Darkly is a remarkable achievement. In elegant, precise prose it captures the personal suffering of individuals during the pandemic while rendering the universality of their condition. Each essay is a version of how any of us could experience these new complexities that will, one way or another, determine all our futures. Dr. Friedberg's practice has been at the epicenter of the pandemic for several months, and he saw it all -- guilt, grief, fear, feistiness, and self-reinvention. He helped people find their way towards some kind of accommodation with post-COVID reality. His account of this straitened reality is, therefore, tempered with possibility and hope. As we make our way through these dark times, this inspiring book will become an essential read.

David Forrest, MD, Clinical Professor of Psychiatry, Columbia University

Through a Screen Darkly is a beautifully-written exploration of the human psyche as it struggles with the uncertainty, anxiety, and fear prompted by the pandemic. It is timely and fascinating. Dr. Friedberg's patients navigate from the initial shock of lockdown, to venturing out into a "new normal," and ultimately to resilience. A Screen portrays the latest incarnation of an ancient human challenge: to psychologically adapt to change and, even better, to learn how to thrive within it.

Heather Berlin, Ph.D., M.P.H., Neuroscientist and Assistant Clinical Professor of Psychiatry, Icahn School of Medicine Mount Sinai

As a literature professor and poet, I am engaged with many issues in this book from perspectives very different from the author’s -- for example, writing poems about Covid-19. So, I’m indebted to this impressive collection for its valuable new insights, notably its readings of literature. And Through a Screen Darkly illuminates our personal experiences of the pandemic -- we’ve seen the movie, now read the book.

I found myself slowing down as I read Through a Screen Darkly because the energy of Dr. Friedberg's personal struggle with his patients' issues became quite palpable. His writing style draws the reader directly, yet subtly, into his own process - an unusual (quiet) slide into the space of his inner work - and I found the immersion quite wonderful. While reading this book I also found myself struggling with resistance to enter into the honesty of the shadows that he delved into with each patient. I moved between struggling with him, and his patients, at various points. A high mark of accomplishment as a writer!

Dr. Friedberg, often informs his readers how he delivers treatment to his patients along with his own thoughts about the process. While he uses the term “screen” for telemedicine, he is attempting in a very real manner to assist and support his patients to be resilient during the Covid-19 pandemic while most all individuals, including Dr. Friedberg were confined to their home.

The book might have been better titled “resilience.” Dr. Friedberg’s effort is to maintain his patients’ ability to cope with life during the pandemic that dramatically interfered with his patients’ lives and their purported goals. This is a book worth reading to understand the process of psychiatric telemedicine.

This book is a must-read for anyone directly, or through a loved one, impacted by the rapid changes and challenges of the current pandemic. With great empathy, Dr. Friedberg shares the stories of dozens of patients who consulted him while the most uncertain and disruptive moments of the pandemic were unfolding. He describes how simultaneously, he was adapting to a new world of "virtual" professional engagement with patients, and the demands of the new environment. His writing is thoughtful and insightful, providing expert guidance on resilience, the ability to recover and adapt to any new situations. Thank you for this gift!

“Right now, you could make 100 right decisions, but if you make one wrong one, you could be dead.” This statement, part of Dr. Friedberg’s advice to a patient to slow down and think things through, exemplifies what many of us who endured the Covid 19-lockdown experienced. Through a Screen Darkly is series of highly readable, mini case histories that are full of intimate, personal exchanges and will engage anyone who had to go through this once-in-a-century pandemic. Readers will see themselves in these essays, but they’ll also see how other people coped with the fear, claustrophobia, isolation, and uncertainty. The patients include a financial wizard, a science fiction writer (“I’ve got writer’s block. I can’t imagine the future anymore.”), a painter (“I’m so damn frustrated … all this virtual stuff is ruining my life”), and, among many others, a laid-off interior designer (“How was I going to help her when she had no work, her friends couldn’t be of any help, and there weren’t a lot of obvious opportunities to go out and be of use?”). In addition to the personal testimonies and stories, you’ll also get great coping strategies from Dr. Friedberg, most of which revolve around the concept of resilience, by which he means an abiding mental and emotional flexibility. As Dr. Friedberg points out, resilience isn’t something critical just to surviving the pandemic; “we should cultivate it for the long haul, learning to be adaptable, to make connections, and to act decisively when the need arises.” We should “think of resilience as part of our personal reserve, like the strength in our muscles, rather than a quick-fix skill.” In other words, Through a Screen is entirely is applicable to our lives going forward, not just to our experience of the past. How often do you find a book that’s a novel record of human experience, an instruction manual for navigating life, and a page-turner all in one?

It is a rare event that impacts our lives at every level, from the global economic and political level to the particular intimate detail of each of our experiences. One result of such a spectrum is the confusion of explanations, of stories and of facts that we are forced to wrestle with, as we try to make sense of our impacted lives. However, Dr. Friedberg has used this very fact to bring light and life to his interpretation of the "year of the pandemic". He has woven together broad themes with poignant and sometimes harrowing personal stories, so that this background and relief form a coherent pairing.
He is careful to point out that this is but one view from many, which will contribute to some future historical view, but it has an implicit coherence, denied to many other assessments, derived from 25 years of psychiatric practice. And, as if to emphasize this, there is an early delightful section of self musing about the role and possibilities of psychiatric treatment in the face of so many disasters. How can he give calm sensible advice to someone whose life is crashing spectacularly into pieces?
Dr. Friedberg's literary prowess enables him to keep the complexity of the discussion under firm control, and I felt that I was sitting in his attic alongside him, struggling to empathize, theorize and make some sense of it all. The result is a very readable account, which also stimulates consideration of profound issues. It makes a real contribution to the inevitable outpouring of books that we can expect in the near future.

For those of us who find it useful to apply a psychoanalytic lens to understanding our difficult times, this is quite a useful and interesting book
