•         Fill out your forms prior to your appointment

If you are a current patient who received an email about Portal Access, use that email address and the provided password to gain access through the Portal Login in the top right corner of portal.

Are you requesting medical records from The Pain Center for yourself or to another provider?  Please complete the Authorization to Release Medical Information and email the completed form to medrec@thepaincenter.com.  

For medical emergencies, please call 911 or proceed to the nearest Emergency Room.
Do not use the portal for any urgent medical issues.
