FT CNA  position working a 64+ status per two week payperiod.  Is requred to have a minimum of one year experience working with the elderly.

 

This is a day position that requires flexibility to include different start times during the day and to work one evening until 9pm a week.  Shift also includes a weekend rotation for "on-call", one weekend every 4-5 weeks.  (All shifts can vary based on client need and staffing needs so flexibility is a must.)

Overview

The CNA-NAR is responsible for providing resident care as required under the direct supervision of the RN or LPN. 

Responsibilities

Performs initial and ongoing data collection on all residents and documents the plan of care results. Completes assigned tasks to ensure timely, efficient and safe patient/resident care. Communicates and interacts with patients, residents, families and other health team members to assure resident safety and well-being.

  • In good standing on the State Nursing Assistant registry or appropriate entity.

  • Experience in health care or long term care.

Job Summary: Promotes and maintains the health of enrolled participants through nursing assessment, treatment, teaching and counseling.  Responsible for completing enrollment and periodic assessments of health needs to contribute to the individual’s plan of care. Responsible for the implementation of nursing care plan and coordination of nursing with other services.   

Primary Responsibilities:  Under limited supervision and in accordance with all applicable federal, state and local laws/regulations:

  1. Completes RN assessment during initial, semi-annual, annual and significant change evaluations
  2. Ensures participants care plan is reflective of all nursing interventions and participant diagnoses
  3. Initiates preventative nursing care procedures as appropriate for the participants’ care and safety
  4. Completes participant cares scheduled within the day center and in participants’ homes as needed
  5. Collaborates with the Home Care Nurse about teaching needs of participants and families 
  6. Implements, directs, evaluates and provides oversight of nursing care and care provided by certified nursing assistants. 
  7. Collaborates with provider to facilitate PACE appointments, participant needs and plans/follow-up for specialty appointments and medical orders
  8. Assists with preparation for interdisciplinary team meetings and ensuring that all updates for participants and needed appointments are added to the schedule 
  9. Communicates regularly with pharmacy regarding prescription changes and ensuring that participant MAR is up to date
  10. Provides care to participants in a positive, nurturing, professional manner. 
  11. Provides health teaching to participants with complex nursing needs
  12. Triages and ensures assessments are completed on participants reporting any sudden illness or onset of illness at home.
  13. Is flexible and able to adjust schedule to meet complex needs of participants.
  14. Consults with other staff members regarding any concerns about appointments, complaints or other anticipated/discovered needs identified during assessments.
  15. Works with social worker regarding family and caregiver in caring for homebound participants.
  16. Maintains accurate records of all nursing services provided to participant in EMR
  17. Participates in Quality Improvement activities as requested
  18. Collaborates with hospital for any participant hospitalized

Job Summary:

Responsible for the daily operations of food service according to facility policies, procedures and Federal/state regulations.  Provides leadership and guidance to ensure that food quality and safety standards are met.

Primary Responsibilities:

  • Coordinates the ordering of daily meal supplies based on particpants' diets and attendance schedules.
  • Manages inventory of food service products and supplements.
  • Monitors receiving and proper storage of food serivce products.
  • Completes meal preparation, service and clean up duties in the day center and/or communicates with Site Director to assign other duties as needed to help with dietary service.
  • Plans and adjusts menues for noon meals and daily snacks a the day center.
  • Manages the operations and maintenance of food service equipment.
  • Monitors diet orders during meal service and food intake of participants and reports concerns to dietitian or nursing as needed.  
  • Enters appropriate documentation into electronic health record for participants.
  • Updates daily diet/attendance shees for the day center and home delivered meals/supplement sheets.
  • Delivers items to particpants homes.
  • Ensures the PACE particpant's dietary plans are followed as written.
  • Knows and adheres to the philosophy of Northland PACE.
  • Participates in Northland PACE committees as requested.
  • Attends and participates in scheduled staff meetings and participant care meetings as requsted.

Education:  Two year associate degree as a Dietary Technician or BS or BA degree in Home Economist from an accredited college or university preferred however, will also consider candate with High School Diploma with two years of experience in a food service setting and successful completion of and approved course in food service management.

Experience:  Minimum of two years in food service setting;  preferred one year working with elderly.

 

Special Training:  Ability to navigate computer systems; ability to cood and bake institutionally; must possess leadership qualities and be able to supervise and secure the cooperation of other personnel, in addition to working well with interdisciplanary team members and particpants.  Ability to adapt to change in a positive and professional manner.  Must meet established competencies for the position.  Must be medically cleared for communicable diseases.  Must have all immunizations up to date before engaging in direct particpant contact.

Physical Demands:  Ability to stand and walk intermittently throughout the day, as well as reach, stoop, bend and lift dietary supplements and equipment.  Ability to see, hear and verbally communicate.

Working Conditions:  Primarily inside; well-lit, climate controlled environment.  Hazards of instutional kitchens and potential exposure to infectious diseases and environmental hazards.

Employment Specifications:

Education:  Associate or Bachelor’s degree in nursing from an accredited college or university.

Experience:  At least two years as staff nurse; home and/or community health preferred.  One year’s experience with a frail or elderly population.

Licensure, registry or certification required:  Current North Dakota nursing license.  Current, valid North Dakota driver’s license.

Special Training:  Must be able to work effectively with an interdisciplinary team.  Must be able to communicate effectively.  Must possess strong interpersonal and organizational skills. Ability to adapt to change in a positive/professional manner. Must possess the ability to effectively handle difficult and unusual interpersonal situations.   • Provides basic personal care assistance and/or supervision (bathing, grooming, toileting, feeding) and assistance with any other Activities of Daily Living (ADLs).
  • Provides assistance with Instrumental Activities of Daily Living (IADLs) across care settings.  
  • Performs homemaking, meal preparation, light cleaning and other household duties, shopping and other chore service for participants.
  • Provides assistance with nursing care, restorative therapies, activities and meals across care settings.
  • Participates in assessing, planning care for and documenting care for participants as member of interdisciplinary team.
  • Knows and adheres to the philosophy and goals of the Northland PACE Program.
  • Maintains a quiet and safe environment for participants, visitors and staff.
