Immunization & Health Documentation

Before entering Colorado College, all new students are required to submit documentation about a number of immunizations and other health screenings (please read the entire page).  This is the only way we will be able to track and accept documents that are submitted.  

Each of the required forms is available to open and print below.

The first 2 pieces of documentation (your Colorado Certificate of Immunization and Tuberculosis Screening Questionnaire forms) through the Admission applicant portal:

  1. The Colorado Certificate of Immunization (required by the state of Colorado), a 2-page document that demonstrates proof of:
    1. MMR (measles, mumps, and rubella) immunization OR a signed statement of immunization exemption based on medical, religious, or personal reasons (separate documents; see below). Please note: you may be asked to leave campus in the event of an outbreak, should you waive the immunizations.
      • Medical Exemption Form must be completed and signed by a medical provider.
      • Non-medical Exemption Form must be completed and signed by the student and/or parent/guardian. This form also REQUIRES either the signature of a medical provider OR a certificate of completion of the Colorado state Online Immunization Education Module.  You can access this education module here.
    2. Meningococcal disease immunization OR a signed acknowledgment that you have reviewed the information and decided not to obtain the immunization (Page 2 of the Colorado Certificate of Immunization). Please note: you may be asked to leave campus in the event of an outbreak, should you waive the immunizations.   2. Tuberculosis (TB) Screening Questionnaire completed within the 6 months prior to arrival on campus with the signature of your health care provider.

The Colorado state Online Immunization Education Module is available 24 hours a day and seven days a week. It can take approximately 20 minutes to complete. In order to successfully move through this Online Module and obtain the Certificate, you must review the Technical Guidance and Troubleshooting Document.”  At the completion of the module, you will be able to complete an electronic form to obtain a Certificate of nonmedical exemption. You can complete this form for more than one child without having to retake the course. Access the education module.
