Oregon Eligibility Guidelines

Everyone 5+ is eligible to receive a vaccine* in Oregon.

*Parental or guardian consent is required to vaccinate people 5–14 years old, but this consent does not require that a parent or guardian be present during vaccination. Written consent may be obtained in advance. 

Youth and their guardians are always encouraged to talk with their physician if they have questions or concerns.

Minor Rights: Access and Consent to Health Care: This resource document for providers, parents, and educators provides basic information about minors’ ability to consent to health care services.

OHA Statement on Minor Consent: This states that OHA supports the rights of minors 15 and older to consent to COVID-19 vaccination without parental consent.

NW Rides.
Non-emergency medical transportation (NEMT) services are a covered Medicaid benefit for Columbia Pacific members. If you have no way to get to a covered health appointment, a rideshare program called NW Rides can get you there. Depending on your needs, you may be provided rides, get passes to ride the bus or get help paying for gas. Contact NW Rides toll-free at 503-861-0657 or 888-793-0439 to arrange your ride. The call center normal business hours are Monday through Friday 8 AM to 5 PM.


Get your employees vaccinated:

Columbia County can come to your workplace and vaccinate your employees and customers. Please consider offering your employees to take time off if they have side effects from the vaccine the following day. Columbia County Public Health is working with partners to offer on-site COVID-19 vaccinations to Columbia County businesses whose employees and families are unable to attend conventional vaccination events. If you are a business in Columbia County and would like to request an on-site COVID-19 Vaccine Clinic for your employees, please complete this survey. Businesses will be contacted by a member of our team to discuss options for an on-site clinic.
