Engaging a top-down and bottom-up approach, PRIME®‘s unique learning models empower healthcare teams and systems to reflect on and react to effective workflow processes, pathways, algorithms, and best practices that improve care quality and patient outcomes. Clinicians, administrators, teams, and patients are engaged in a community of learning to design, react to, benchmark, monitor, and measure action plans based on group interventions. Designed to advance collaborations, PRIME®‘s systems-based learning interventions have demonstrated sustainable and scalable behavioral change.

Performance Improvement

Performance Improvement

In recent years, PRIME® has supported thousands of clinicians in acquiring essential performance-related knowledge, competencies, and skills across chronic and acute diseases. Performance improvement modules (PIM) meet requirements for maintenance of certification (MOC) among medical specialty boards who certify physicians practicing in specialty medicine. Practice-based assessments are also applied for physicians-in-training through PRIME®‘s graduate medical education (GME) platform at GMEToday.org.

Patient Engagement

PRIME® embraces patient engagement and learning as a vital cornerstone in a successful population health strategy. Utilizing a collaborative learning solution, clinicians and patients/caregivers participate in aligned experiential learning opportunities across multi-channel educational formats. Educational designs are fed by data results from tethered patient and provider surveys, which display mismatches in goals of care. Through AfterMD.com and other patient resources, PRIME® also provides patients and caregivers with point-of-care tools, surveys, and other evidence-based guidance to assist them in their healthcare journey.

As the leading accredited provider in research-based quality improvement education, PRIME® advances the science of learning and behavior change. PRIME®‘s investigators work with our collaborators to measure and evaluate practice patterns and care outcomes for patient populations across health care networks and settings. The organization designs, implements, and executes research methodologies and studies with a goal to impact population health outcomes.

Public Health Priorities

Long-standing contracts with the Patient-Centered Outcomes Research Institute (PCORI) and the Agency for Healthcare Research and Quality (AHRQ) have assisted PRIME® in appreciating and mitigating health disparities among patients in underserved populations. Access to care, inequity in care, and barriers to health literacy have profound impact on the health care macro system. With a goal to impact public health priorities and national healthcare initiatives through sustainable learning solutions, PRIME® partners with key organizations to address population health issues.

Care Coordination

Care Coordination

With a 23-year history of providing interprofessional continuing education (IPCE) across federal, military/VA, payer, provider, academic, and other health care settings, PRIME® engages clinicians, patients, healthcare teams, and systems in improving care coordination to achieve sustainable results. PRIME® is supporting community health centers and clinics, addiction centers, institutional centers of excellence, and entire health systems in better connectivity and communication, reduction in variations in care, increased information exchange and interoperability, and better transitions of care.

Quality Improvement

As essential tools in QI programs, quality measures enable clinicians and healthcare organizations to quantify the proportion of patients who are managed according to evidence-based recommendations and guidelines. While conducting QI educational programs using core measures since 2006, PRIME® launched its integrated quality of care education (iQCETM) model in 2013. The goal of iQCE is to leverage real-world data analytics with patient and provider tethered engagement to achieve sustainable and scalable educational impact. Our results have been published in numerous peer-reviewed publications demonstrating positive impact on the healthcare ecosystem.
