Did you know HPV vaccines are important for oral health and can protect against certain kinds of oral cancer?
HPV vaccine (Gardasil 9) will be offered to participants age 9-18. Please follow the steps below to register.
Other routine immunizations will also be offered to get up to date for the 2023-2024 school year. You must register to attend and you will need to bring printed vaccination records with you. Please check with your school nurse if you do not know which vaccines your child needs. Vaccination records will also be reviewed prior to receiving immunizations at the clinic. Please follow the steps to below register for the clinic.
If you have one of the following insurances, please bring a copy of your insurance card and driver's license. Immunizations received will be billed to your insurance.
• Aetna PPO
• Blue Cross Blue Shield PPO
• Cigna PPO
• Humana PPO
• United Health Care PPO
• Medicaid or CHIP
If you do not have an accepted insurance there is no charge for the vaccines thanks to a generous grant from MD Anderson Cancer Center.
After you register you will receive a confirmation containing a link to a 3 page consent form. You only need to print out one set for vaccines. Please bring these, filled out along with your photo copies, vaccination records to the clinic. This will ensure the shortest possible wait. Consent forms and copiers will also be available at the clinic if you are not able to print this beforehand.
Registration and previous vaccination records are required to attend. Walk-ins cannot be seen.
