Please read below information carefully before registering your student(s):

This after-school program is FREE and available to students in grades 3-5. Students will be enrolled on a first come first serve basis, with a maximum of 26 students total (two teams of 13 students each). Practice will take place on Thursday BEFORE school from 7:40am-8:40am starting on September 13 and ending on October 18. The club will be lead by SAM Soccer staff members from the Germantown Soccerplex and will take place on the soccer field behind the school. Students MUST have transportation to school for practice- here will be no transportation provided. All students must arrive on time, no later than 7:40am, or they will no longer be permitted to attend the after-school program. In the event of inclement weather, a staff member will contact parents with cancellations. Please complete one registration form for each student you would like to register for the program (if registering a sibling, do a seperate form for each child). *Soccer cleats and shin guards are recommended but not required for this program.
