This application form has six sections, outlined below:
- Basic personal information (name, DOB, etc.)
- Academic/professional history (university attended, work history, etc.)
- History with Japan/Japanese studies (travel to Japan, study of Japanese, etc.)
- Other information (other travel abroad/languages spoken, hobbies, future goals, location preference)
- Medical assessment (height/weight, blood type, current medications, etc. Answers in this section will not affect hiring decisions, but are required to process the application.)
- Document submission:
- Short Statement of Purpose essay outlining what you hope to contribute to the Program as well as what you hope to take away from it (max. 2 pages, double-spaced)
- Digital copy or scan of your official university transcript(s)
- Scan of your diploma or an official letter from your university certifying your expected graduation date
- FBI Identity History Summary Check (for applicants currently residing in the US only)
- Two letters of reference from teachers, supervisors, or other mentors
