This form will be used to collect the data for the DP Self Study. The self study forms the basis of a school's evaluation process and helps to set the school's agenda for program growth over the next five years.

Parents will complete the sections covering the schools' philosophy and the implementation of the programme.
The numerical scales refer to levels of implementation, with one being "low" and four being "high." The descriptors for each level philosophy and organization.
