To register Athletes & Unified Partners in your delegation for the Unified Bocce Tournament, complete the form below.  No more then 5 teams per Delegation. If you enter more, after the first 5, the remaining teams will be entered on a first come first serve basis until space has been filled.  Delegations are not required to provide a qualifying score.
 
You will need the following information to complete this registration process:
       o HOD Name & Email
      o Covid-19 Response Person Name
       o Team/s with 2 Athletes & 2 Unified Partners*
       o      Athlete: Gender, Date of Birth
       o Unified Partner: Gender, Date of Birth
 *If you need a student from Maine Maritime Academy to participate as a Unified Partner with your team please indicate this under "Additional Comments." MMA students are limited to no more than two per delegation.

At any time during this process you can always select the "Back" button (bottom of the form, to the left of the "Next" button) to go back and review/edit the data that you completed prior to your current section.  Once this form is completed entirely and submitted (click "Submit" on the last section) the HOD will receive a confirmation email (to the email provided) with a PDF of submitted information.  Changes to what your Delegation submitted can be made up until 2 weeks prior to the event, the sooner the better!
