To register athletes in your delegation for Option A of the Virtual Bocce, complete the form below.
In Phase 1, NO MORE THAN 10 TOTAL PARTICIPANTS (including athletes, coaches, volunteers) are permitted during training sessions or during competition.
Please indicate if you will use your home court or the community court in Hampden. If you will be using the Hampden court you are required to sign up for a time block. If you have more than 10 participants you will need to reserve another time block by submitting this form again. Time slots will be filled first come first served. If there is a conflict with another team you will be emailed to discuss a second choice.

Time Blocks Available:
Please select your date (April 19-30th) and write in your preferred one hour time block.

You will need the following information to complete this registration process:
       o HOD Name/Email
       o Name of COVID 19 Response Person for each competition time/time block
       o Athlete: Name, Gender, Date of Birth, Event (Qualifying Score not required)

We ask that all entrants on this page have a completed COVID-19 Code of Conduct & Risk Assessment form and Communicable Disease Waiver on file at the state office prior to the start of the event. Athletes are also required to have up to date medical and consent forms on file.

At anytime during this process you can always select the "Back" button (bottom of the form, to the left of the "Next" button) to go back and review/edit the data that you completed prior to your current section.  Once this form is completed entirely and submitted (click "Submit" on the last section) the HOD will receive an email (to the email provided) with a PDF confirmation of what was submitted.
