Training Dates:  Starting April 6th-April 29th, Tuesdays & Thursdays, 11-12pm, 2-3pm, & 3-4pm

To reserve a pool time block for your delegation, complete the form below.
 
You will need the following information to complete this registration process:
       o HOD Name/Email
       o Dates and times of desired sessions
       o Number of athletes participating
       o Name of Covid Response Person/Point of Contact during each session

Teams may register in groups of 5 total. Two teams of 5  will be permitted in each 1 hour time block. If you have more then 5, you may reserved both sections ("a" and "b") for the time block. In accordance with the R2P Guidelines of Phase 1, at no time can we exceed 10 Special Olympics participants (including athletes, coaches, staff, spectators). All available dates and times for April will be listed. If the time block says "RESERVED" please do not select this time.

At anytime during this process you can always select the "Back" button (bottom of the form, to the left of the "Next" button) to go back and review/edit the data that you completed prior to your current section.  Once this form is completed entirely and submitted (click "Submit" on the last section) the HOD will receive an email (to the email provided) with a PDF confirmation of you what was submitted.  Please Email Christina Ehrhard, pphtina@somaine.org, with changes.

Athletes must have a current Athlete Medical and Consent/Release on file with SOMaine prior to training. All athletes, coaches and staff are also asked to have signed Covid Code of Conduct forms.
