You will need the following information to complete this registration process:
       o HOD Name/Email
       o Name of COVID 19 Response Person during event
       o Athlete: Name, Gender, Date of Birth, Event and Qualifying Score for Speedskating events and School age Snowshoe events.

We ask that all entrants on this page have a completed COVID-19 Supplemental Form and a Communicable Disease Wavier on file at the state office prior to the start of the event. Athletes are also required to have up to date medical and consent forms on file.

At anytime during this process you can always select the "Back" button (bottom of the form, to the left of the "Next" button) to go back and review/edit the data that you completed prior to your current section.  Once this form is completed entirely and submitted (click "Submit" on the last section) the HOD will receive an email (to the email provided) with a PDF confirmation of what was submitted.
