Complete the information in this form to register your Delegation's Head of Delegation (HOD), Certified Head Coach and Assistant Coach(es). Athletes will be captured on the Athlete Registration forms.

You will need the following information to complete this registration process:
-HOD Contact Information
-Certified Head Coach Contact Information
-Assistant Coach Names (this is anyone that will be registering with your Delegation that is not an Athlete, Unified Partner, HOD or Head Coach)
-Vaccination status of those attending

All entrants on this form are asked to complete the COVID-19 Participant Code of Conduct & Risk Assessment Form. Links can be found in the bowling registration email and on the somaine.org website.

At anytime during this process you can always select the "Back" button (bottom of the form, to the left of the "Next" button) to go back and review/edit the data that you completed prior to your current section.  Once this form is completed entirely and submitted (click "Submit" on the last section) the HOD will receive an email (to the email provided) with a PDF confirmation of what was submitted.
