PRM Afternoon Transportation Change

This online form MUST be submitted by 3:00 pm on the date the transportation change is needed. Any submissions after 3:00 pm will not be honored and your child will be sent home via the usual mode of transportation. Do not email or text your child's teacher requesting an afternoon transportation change. Submit this form for each child who needs the transportation change.

Do not submit a change for an upcoming day as the submissions are sorted by date.
