This Mental Health Association Oklahoma form is designed for a mix of public use and reporting with Tulsa Police Department and security agencies so that the Association's Homeless Outreach and Rapid Response team can respond to individuals experiencing homelessness.

TPD USE:  For TPD requesting on-site assistance, after radioing for dispatch please fill out form and select RAPID RESPONSE. For after hours or reporting outside catchment area select HOMELESS OUTREACH and fill out form.
