DUE TO COVID REGULATIONS, WE ARE CURRENTLY ONLY OFFERING EQUIPMENT RESERVATIONS TO ORGANIZATIONS THAT HAVE RESERVED A DEPARTMENT OF WELLNESS FACILITY.

This form must be completed and returned NO LATER THAN SEVEN (7) BUSINESS DAYS PRIOR TO THE SCHEDULED EVENT. At this time, reservation requests will only be accepted for events that are being held at Colvin Recreation Center facilities. This includes Western Fields, Willis Fields, the Colvin Fields, and the Tennis Courts. Completion of this form does not guarantee approval. This form is only for equipment rental. For facility rental, please contact the Colvin Center Main Office.
