Transcript Course Addition Form
All students must submit this form along with the Lambert HS Decision to Accept or Deny Course Credit form signed by student and parent, to add courses to their official transcript if the courses were taken outside the Lambert High School schedule. If the course was completed through GAVS, the grade will be reported to your counselor and you only need to complete this form.  If the course was completed through any other online school, a formal grade report must be submitted from the institution through which the course was taken. The official grade report must be submitted to the LHS Counseling Secretary at kfuerst@forsyth.k12.ga.us   FVA grades are reported to Lambert directly from Forsyth Virtual Academy, so no need to complete this form or provide official grade report. Dual Enrollment grades are also reported to Lambert directly from the colleges so no need to complete this form or provide official grade report.
