This General Incident Reporting Form should be used to report nonacademic violations of University policy, including but not limited to alcohol or other drug violations, disorderly conduct, vandalism, and theft of property, that do not involve sexual misconduct or acts of discrimination. This form can be used to report incidents involving individual students or student organizations.
Please provide detailed information regarding the incident you are reporting. Reports made using this form are referred to Student Accountability, Community Standards and Academic Integrity for review. For more information about the student accountability system, visit the Student Handbook.
If this is an emergency and you need to report an imminent threat to or life-endangering situation involving a student, please call the Vanderbilt University Police Department at (615) 421-1911 or 911 prior to completing this reporting form. The General Incident Reporting Form is not designed as an emergency response notification process.
To report a student of concern, sexual misconduct, discrimination, an Honor Code violation, or acts of hazing, please utilize one the following reports:
- For a student of concern, please make a report using the Student of Concern Reporting Form.
- For sexual harassment or sexual misconduct, please make a report using the Title IX Office’s incident report.
- For acts of discrimination, harassment, and/or retaliation, please make a report using the Equal Opportunity and Access Office’s incident report.
- For Honor Code violations, please make a report using the Undergraduate Honor Code Incident Reporting Form.
- For acts of hazing that may violate University policy and/or Tennessee state law, please make a report using the Hazing Incident Reporting Form.
