This tool is designed to help you share your experiences with bias, discrimination and harassment so that we can support you and better understand the complex needs and challenges facing our campus community. It is for use by all members of the UIC community, including students, faculty and staff. If you fill out the form, it will be reviewed by members of the Bias Response Team. Reports involving faculty and staff will be referred to and/or reviewed in coordination with Office for Access and Equity and the Office of Diversity.
You may submit the form anonymously. If you choose to do so, keep in mind that there will be no way for us to follow up with questions, so please take care to provide sufficient detail. If you do include your contact information, a member of the Bias Response Team, which includes members from the Office of the Dean of Students , Office of Diversity and/or the Office for Access and Equity will follow-up with you.
Please note: We strive to respect your privacy as much as possible, however, at times in order to comply with UIC policies, we may be required to share these reports.
