The Dean of Students Office encourages faculty to use this online form to submit all relevant materials, including:
- A copy of the course syllabus;
- All materials related to the alleged violation;
- Any relevant communications between the faculty and the student
All information shared and submitted on this form will be reviewed with the student in question.
