This application has multiple sections, and should only be submitted ONCE. Make sure you have the information to answer each question before starting to fill out this application.

Application Steps:
1. Gather this information: Your contact information, your current ELA Grade, names and email addresses for two teacher recommendations.
2. Fill out this form and submit. The Essay Response should be a 5-7 sentence paragraph.
3. Email Mrs. Mundy, introducing yourself and letting her know that you are interested in joining the yearbook staff or the journalism staff (jmundy@forsyth.k12.ga.us).
4. If your application and introduction letter are acceptable, you will be contacted to set up an interview. To join the yearbook or journalism staff, you must have a short, in-person interview with the adviser (Mrs. Mundy-Yearbook, Mrs. A-Newspaper).
